A person can use this form to request access to a record held by a public body. The requester must provide personal details, describe the record, and pay the applicable request fee unless an exemption applies.

Complete the form for the Information Officer/Deputy Information Officer, provide the requester’s details and contact address or fax number, describe the record and its reference number if known, attach proof of capacity where applicable, and pay the request fee or state the reason for exemption.
