Correction of errors in Identity Document
If your Identity Document contains incorrect personal information, you can apply to have it corrected and replaced free of charge by the Department of Home Affairs. You must submit completed BI-9 and BI-309 forms, proof of the correct information (such as a birth certificate), and two identical colour photographs (unless applying at a smartcard office, where photos are not required).
